Split app into med and base

This splits the hcp/pat variants into base and med versions. In the End we will have 5 variants: repose, pat, pat-med, hcp, hcp-med.

There is no new content, things only got reorganized. In the future we will have to keep track of the legal texts for the different variants separately or try to share content as much as possible to keep the maintenance needs as little as possible.

I also reorganized the features.module.ts files for the various variants: I separated main menu entries and home page entries; both got their own respective files.
