Use values of reminder and schedule time in symptom check setup

About the issue

The schedule and reminder time were in fact being saved, it's just that these values were being ignored when selecting a template in the questionnaire setup

about the fix

I changed the questionnaire setup to set the values from reminder and default schedule if they're present, this should apply for all of:

  • Monitoring setup
  • Creating symptom checks
  • Editing patient symptom checks
