Draft: Prepare ItemAction Factory for FHIR download

FHIRModule now provides an ItemAction for downloading FHIR format JSON.

UPDATE: I copied the changes to action.service and action.commons to #244. Please review over there first, then I'll make a new MR for this issue with only the FHIRModule changes.

UPDATE 2: The new MR is !195 and contains the cherry-picked changes to FHIRModule etc. Closing this MR.
