Dropdown hover states

• I know we need to refactor dropdowns - so if this is involved in that we can save this issue until we do the whole dropdown functionality refactor work

Current: When the dropdown is open: the first entry has a hover state even when the item is not hovered. This means there are 2 items in the hover state at the same time: not in the medication dropdown but in the template/question catalogue filter/question options dropdowns.

Desired: on opening of the dropdown: none are hovered unless the mouse is over the entry
