BUG: leaving self-monitoring / template options

Current: When a user leaves a self-monitoring that is being edited/created, we have a modal to warn the user that all work in the setup will be lost. -> user presses ja and navigates to other part of app. On starting a new self-monitoring, all the questions/settings are cleared but the template dropdown only has the option that was selected in the last half started self-monitoring (and custom templates).

Desired: template dropdown has all of the template options
