Background
The redesign of setting up a self-monitoring form was done for a specific PAT, now we want to use these redesigns for the creating of a custom template. The HCP may use an already existing template and build onto this.
Where to start
The nav bar entry vorlagen opens up a list of templates (i.e. depression, leer, psychose). On clicking any of these templates and the icon [+] the template editor is opened. We want to reuse the set-up a self monitoring layouts for this template editor.
Description of editor page

Header text should be the template title i.e. "Vorlagen - Depression" instead of "Fragebogen HCP13123"

Template dropdown is not on the page: it is not needed for the editor as we start from selecting the template from the list of templates page

Symptom questions are displayed in the symptom question column (if they exist in the template)

Medium action buttons at the bottom are the same

On selection of "follow-up vorschau" HCP is taken to preview page
Description of Preview page:

Header uses the name of the template i.e. "Vorlagen - Depression"

Medium action button "Follow-up teilen" is instead "Vorlage speichern" to save the template

This saves the template into the list of templates (i.e. in the self-monitoring template dropdown list)

The template is placed in the "Benutzerdefinierte Symptomchecks" list on the Vorlagen page after saving

Notes are shown on the preview page (or not whichever is easiest) but when the template is used only 1 notes question should be within the questionaire and at the end of it (last question)
