Leave Self-Monitoring Setup Modal

Background

The HCP is setting up a new self-monitoring with a PAT and is still editing but accidentally scrolls back, presses back, the logo etc.

Concept

When leaving the self-monitoring set-up and preview page we need a modal to ask if they are sure they want to leave and lose their progress.

This is a "critical decision" confirmation modal with HCP primary/secondary colors (see gitlab issues 424 and 511).
