Update Data Viz: HCP/PAT header text

Background

The new data visualization has been added as a separate page for both HCP and PAT variants. A header text needs to be added for the PAT and HCP variants.

PAT Variant: This text is static and doesn't change - see attached image

• "Deine Antworten in der Übersicht"

HCP Variant: This text is not static but uses the session information to identify which PAT data the HCP is looking at - see attached image

• "Therapieverlauf PAT 123456"
• "PAT 123456" is the filler text for the open session id to be displayed
