Apply template questions after template was selected

Summary

Currently the user is able to select a template from the dropdown list

Once the user has done this, the questions from the template should appear in the monitoring setup page, and previously added questions should be removed

Acceptance Criteria

• When I select a template, the questions from the template should be visible in the monitoring setup
• When I select a template, previously added questions should be removed
