C_004_R9 Notes Order for Multiple Therapy Follow-ups

Notes should always be last: if multiple therapy follow-ups are on the PAT interface, please order the notes last.

Currently, when a PAT has multiple therapy follow-ups, the notes is placed at the end of the first therapy follow-up questions, we would like it to be at the last question card in the day-query-run all the time.
